I did go through the process suggested after being asked to provide the PUK code (ie got the proper code from AT&T). When I enter it, I am then asked to "enter new PIN code". I enter a code. Then I'm asked to "verify new PIN code". I enter the same number. Then I'm locked out again, and asked for the PUK code all over again!

My phone puk code got locked. i can unlock the puk code but it says enter new pin code. and when i verify it it says code error
I got the same problem like you did. I just called the customers service and asked for puk number. They also tell me that to use 1234 for the new pin number.

The card is totally blocked because you have been trying PUK codes before you got the code. !
Solution : Replace the card

Test using a different compatible SIM card, since the PUK code lock belongs to the SIM and not to the phone, the new SIM will be initially unlocked, or will ask its own PIN code.
If the new SIM gets locked by itself, then the problem is the phone. If the problem does not occur with the new SIM, the original SIM card was defective.
If the problem happens again with a brand new SIM, then you will need first to reset the phone to factory settings. If still no luck, reinstall the phone firmware. See link below on firmware update. If you go for the upgrade, ensure that you download right firmware. Read instructions and perform procedure as explained. If done in the wrong way the firmware update may damage the phone.
