We had a power outage/surge and after that received the Personal Code Needed message. Did the QV and 9 code and that worked. But need to enter that for each input and each time the TV is turned on. Any other ideas to get the code reset ?

It's 1111. Well, For mine at least. If you get a universal remote and go into menu and turn the channel lock off (It will ask for the code when turning it off) Try 1111 and all the inputs should work without asking for the code each time.
