WHAT IS INITIAL PROGRAMMING
Initial programming occurs when your phone needs a second roaming based and if you accidentally change the NAM settings of your phone to NAM2. the default NAM settings of your phone is NAM1.
Dial *228 and press send. Listen for the prompt and press 1 to program
your phone. Remain on the call while your phone is programmed (could
take up to 2 minutes). Do not hang up until you hear, 'Your phone has
been programmed successfully."
If that does not work call 1-800-922-0204 from another phone.
If you have a straight talk phone then to activate your new pre-paid services after choosing a plan and paying for it is to dial the following number: *22890 (yes you will need to first dial star and then the number)

Call your service provider to have it reprogrammed.
This means maybe someone deleted the basic programing features of your phone like phone number. or the system ID got bumped by a software error. or it could be that the phone just got flashed. i know the code to program the phone but if you don't know about internal programing it will do more damage than good. since it will kill your phone if any wrong settings is applied.
Best bet is to have your service provider tell you the programing code and settings to be changed. because they will be telling you over the phone what to do.
